Admission Process:

1. Fill out the online assessment. This assessment is sent to Heather at Capstone, once it is received, she will call you to answer questions and concerns.  Please feel free to call at anytime toll free at 866-729-4479 to ask questions and discuss concerns.  Heather is available on Monday thru Friday 8:00am – 5:00pm CST.

2. Based on the assessment and phone conversations, it will be determined if Capstone is appropriate for your family’s needs.

3. If admission is chosen as the next step to take by Capstone and your family, then Heather will inform you of dates when an admission is available.  Depending on Capstone’s availabilities and your family’s schedule, a date and time will be set up for an admission. Admissions times are Monday thru Friday at 9:00am and 2:00pm.  Wednesday’s admission time is at 9:00am only.

**If openings are not available immediately, you may reserve a future

opening.  In this case, a $1,000 deposit is necessary to secure that space on the waiting list.  The deposit is refundable up to 14 days prior to admission.  This money is credited to your account towards the initial payment.

4. Once an admission time has been set, Heather will e-mail directions and other pertinent information.

5. Once an admission has been set the admission packet can be printed, signed in appropriate places, and mailed to Capstone or brought with you on admission day.  This is not required, however it is convenient to have the paperwork completed prior to the admission to allow more time to tour the facilities, meet with your therapist, and have time reviewing questions and concerns.  If you have time prior to your admission, it is suggested that you take the time to complete this.  If you do not have printer access, then a hard copy can be mailed to you.  Also if you are mailing it prior to admission, a postage paid envelope can be sent to you.  Please contact Heather if these things are needed.
