Capstone Ministries recognizes that many families do not have the resources to access quality residential treatment without financial assistance. It is our goal to serve as many young men and families as possible, which makes us committed to assisting you as you decide if Capstone is the best place for your son.

Before coming to Capstone, we will provide you with information regarding estimated or actual charges for our services. Additionally, as services are rendered, we will file insurance claims, for allowable and reimbursable charges. If you determine that additional funds are needed to assist in paying for treatment, then you may apply to the Capstone Charity Care program for a scholarship.

To apply for financial assistance through the program, you will need to complete the following:

1. Complete the application form found here: Financial Assistance PDF
2. Provide income verification for all wages, disability benefits, workman’s compensation, alimony, child support, and any other form of income. Income can be documented by submitting the following:

  • 60 days of recent payroll stubs
  • Letter from Social Security Administration or your state’s Department of Labor regarding governmental benefits received
  • Letter of support from individuals providing for the resident’s basic living needs
  • Proof of dependents
  • Proof of child support and alimony
  • Interest or dividend statements.
3. Provide copies of the last two years’ tax returns.

Once completed, the application, income verifications, copies of tax returns and personal financial statement should be sent to the Director of Finance & Operations at the following address:

Financial assistance is awarded on a sliding fee scale based on income levels, family size, and net worth, as funds are available, pending IRS approval of our tax-exempt application.

You will be considered eligible for application to the Capstone Charity Care program if:

1. You meet all financial requirements; and
2. You are underinsured, or
3. You are uninsured.

You will be considered ineligible for program if:

1. False information was provided by the resident or responsible party; or
2. The resident or responsible party refuses to cooperate with any of the terms of this policy; or
3. The resident or responsible party refuses to apply for other programs after it is determined that the resident or responsible party is likely to be eligible for those programs.

Eligible Services of the Capstone Charity Care program are all Capstone charges deemed medically and therapeutically necessary. Any services that are not medically or therapeutically necessary and any discretionary charges are ineligible for payment under the Capstone Charity Care program.
