The goal of this phase of treatment is to help in the transition home, anchor the family to the contract and game plan, and for the resident and family to work for two years on the “titanic turn”. The specific goals within this phase are:
- Weekly aftercare sessions with your Capstone therapist for 3 months
- Attending Capstone family reunions (these are held twice a year)
- Adhering to the contract and game plan
- Calling and asking for help when needed
- Commitment to the 12 T’s
- Tortoise on a triathlon mentality (slow and steady)
- Phases of game plan
- Utilizing relapse prevention strategies
- Continuing to grow in being their true selves and in understanding of their purpose
Length: 2 years post graduation
Description: This is the phase following treatment. It is a two year phase that includes following the behavioral contract and game plan. The phase includes aftercare therapy, family reunions, and a commitment to finishing strong.
- Capstone Aftercare Therapy: The resident and family’s individual therapist while in the program continues phone therapy once per week for three months (there is no cost for this service).
Capstone recommends that families have a “face to face” therapist closer to home prior to graduation. This therapist will be the sole therapist at the end of the three months post graduation. During this time of overlap, the Capstone therapist is able to work with your therapist at home to help with the transition.
- Family Reunions: Capstone invites graduates and their families within two years of graduation to come to a fall and spring Family Reunion.
- The fall Family Reunion is in October and the spring reunion is in March. Reunions begin on Friday evening and end on Sunday afternoon.
- Capstone rents a retreat center near Capstone with sleeping, eating, and activity arrangements.
- Each family gets a two hour therapy session with their individual therapist, a two hour small multi family therapy session with the director Adrian Hickmon, three other two hour multi family therapy sessions with Capstone therapists and staff, a Sunday morning worship service together, and many hours of visiting and connecting with other Capstone graduate families.
- The cost for a reunion is currently $800 total for the family and includes sleeping quarters, meals and snacks, 8-10 hours of therapy, examining the game plan, and lots of fun and encouragement.
- Finishing Strong: The family follows their Game Plan with the support of their face to face therapist and informal contacts from Capstone via email, etc.
